The origin of the hamburger menu icon

I'm sure you've seen it around. In fact, if you're browsing this site on mobile, you probably see a form of it at the top to navigate through my menu. It's dubbed the 'hamburger icon' or the 'hamburger button'. But have you ever wondered who designed it and how it came to be?

An Evernote post explains a bit further saying that the menu icon we know today was first introduced on the Xerox Star personal workstation. The system's user interface was designed by Norm Cox. But despite its ubiquity in many popular websites, many users are unfamiliar with the buttons function. That's...kind of sad.
